Diagnostics

Management

  • Semen abnormalities: This includes decline in sperm number, decrease in sperm motility, and increase in abnormal sperm morphology.
  • Male genital tract infection: This can affect semen quality.
  • Reduced levels of fertility hormones: Replacement therapy is offered.
  • Sexual problems can cause delayed fertility.
  • Absence of semen: Due to failure of ejaculation or retrograde ejaculation.

  • M-TESE & TESA: Microscopic testicular sperm extraction (m-TESE) is done for functional azoospermia cases. The use of a surgical microscope increases success rates. Testicular sperm aspiration (TESA) is done for obstructive azoospermia and aspermia cases. It reduces the rate of complications due to unneeded open surgery.
  • Microscopic varicocelectomy: Removal of varicocele in properly selected patients improves semen parameters and pregnancy rates.
  • Epididymovasostomy and vasovasostomy: This can lead to the presence of sperm in semen in obstruction cases.

  • This is done in cases in which males want to preserve fertility and fix issues that may affect their semen. This includes chemotherapy, radiotherapy, some surgeries and progressive decline in semen characteristics, count and / or motility.

Sexual medicine

Initial consultation includes history taking, complaint courses, and progression, followed by a physical examination. Further testing is done that includes imaging (penile duplex and ICI), blood tests, and in some cases, more advanced investigations may be ordered.
